SUPPORTED SERVICES
Provides participants with the opportunity to live independently in their own apartments or home, with individualized support tailored to their specific needs, goals and choices.
The Supported Apartment Service provides participants with the chance to reside independently in their own apartments or homes - with resources in place for successful community living.
Within this service, each participant has an assigned MCHA counsellor who works with them, their family and their community care team to identify their personal needs and goals. Support plans and hours of service are tailored to each participant and adjusted as needed. Some examples of areas we can provide support in include:
budgeting and money management
grocery shopping and meal planning
home maintenance
psychiatric and other medical concerns
community access and social skill building
problem solving and crisis management including access to a staffed 24/7 emergency response service.
Participants also benefit from joining MCHA's various activities such as movies, annual picnics and holiday parties. Celebrating accomplishments is an important part of our service.
The Supported Apartment Service staff are competent and dedicated professionals who work in accordance with MCHA's comprehensive code of ethics.
